Potential Indonesian herbs to develop an immunomodulator herbal mix supplement: a hermeneutic narrative review

Abstract


Introduction: Well-maintained immune system is important to fight against pathogens. Natural immunomodulator agents derived from herbal plants, are now being on demand, especially during Covid-19 pandemics. This review aims to search for Indonesian herbs which were mostly studied as immunomodulator agents and to identify their immunomodulator potencies. Altogether, the findings hopefully may lead to propose a novel combination of Indonesian herbs as potential immunomodulator herbal mix supplement.

Method: This study is designed as a narrative review with hermeneutic approach. Literature searching was conducted using Boolean operator and citation tracking in Google Scholar database from 2011-2021. Relevant articles were identified to list the most Indonesian herbs studied as immunomodulator agents. All immunomodulator studies related to those listed herbs were analyzed based on PRISMA guideline. Quality assessment for selection studies was performed with JBI instrument.

Results: Database searching and citation tracking hit 574 articles, of which were screened for relevancy and duplication, resulting to 261 studies. Among those relevant studies, the five most immunomodulator herbs studied, were black cumin (Nigella sativa), meniran (Phyllanthus niruri), ginger (Zingiber officinale), sambiloto (Andrographis paniculata) and red betel (Piper crocatum). Based on the quality assessment, 26 studies were selected for data extraction related to those 5 herbs. It revealed that their immunomodulatory potencies are mainly due to pro-inflammatory, antioxidant and phagocytic activator properties.

Conclusions: There are 5 potential immunomodulator herbal plants, of which three herbs namely meniran (Phyllanthus niruri), ginger (Zingiber officinale) and sambiloto (Andrographis paniculate) could be further developed as one potential immunomodulator herbal mix supplement.
